If you do not have a copy of the patient's ID card but know which PPO network the patient accesses, you may view the claim submission information below by selecting the appropriate PPO network. In order to process claims more efficiently, we ask that you submit your claims electronically using the EDI Payer ID whenever possible. Please note: Claims coming to us from Medicare are considered crossover claims, so submitting a paper claim results in unnecessary duplicate claims.
