The Extra Care Notification Program offers you and your family a source of support when you are facing complex medical care needs.

How it Works

1. Simply call the Extra Care Notification Program toll free at 1-866-539-1696 to speak with one of our registered nurse case managers.
2. You'll be asked to provide information about a current or future hospital stay for you or a family member.
3. Your nurse case manager will work with you and your health care providers to coordintate services regarding any special needs you may have during or after your hospital stay.
